Please tell us if you change your address, and/or telephone number, changes of Address Forms are available in reception. If you are moving outside of the practice area you will be advised if you need to register with a new practice.
The practice accepts all new registrations from patients living within the practice area. There is no discrimination in terms of medical need, the need for home visits and/or medication requirements. Patients who have been previously registered and still living within the practice area can re-register with the practice. Patients who have been removed from the practice list due to behaviour (including multiple missed appointments) will not be able to re-register. All new patients will be requested to provide proof of address. This may include a utility bill (mobile telephone not included), rent or mortgage agreement, council tax bill, or solicitor correspondence – all must be dated within the past 3 months.
Patients can utilise a variety of online access providers (NHS App, Rotherham Health App for example) to undertake the following
Patients should create an NHS Login online which will allow them to access their chosen online access provider.
Patients will be able to view their Prospective medical record once the app is created – meaning that you will be able to see what’s in your medical record after the date of activation. You will not be able to see your historic medical records
(Please note that in certain circumstances patient access to their medical record may be withheld or restricted where potentially sensitive, or harmful, information may be held within the record. If you cannot see your medical records created after activation then please contact the practice.)
Proxy Online Access is also available in circumstances where a patient cannot manage their own online access. This may be due to age, or health needs, which prevent them from using online access. In these instances access may be given to a proxy user to access them on their behalf.
For children Under 13, proxy access may be given to a parent. Access will be granted up to their 13th birthday.
For Children aged 13 – 15 the child themselves will need to authorise a proxy user to access their medical records. Forms can be requested from the practice for this purpose.
All patients aged 16 and over should have their own online access excepting where medical reasons may restrict this. Please contact the practice if under these circumstances if you still wish to have proxy access to another patient for further advice on options.
Violence and abuse to any member of our staff will not be tolerated, and will result in patients being removed from our practice list.
We operate a zero tolerance policy towards violent and abusive behaviour. Unacceptable behaviour that causes harm or the fear of harm to any person within the surgery may lead to the removal of such a patient from the practice list onto the Potentially Violent Patient Scheme.
Many complaints can be resolved by an informal discussion with your Doctor or our Practice Manager. However, we also offer a formal practice complaints procedure and our Practice Manager can give you further information. We always try to provide the best services possible and we try to deal swiftly with any problems that may occur.
