Please tell us if you change your address, and/or telephone number, changes of Address Forms are available in reception. If you are moving outside of the practice area you will be advised if you need to register with a new practice.
The practice accepts all new registrations from patients living within the practice area. There is no discrimination in terms of medical need, the need for home visits and/or medication requirements. Patients who have been previously registered and still living within the practice area can re-register with the practice. Patients who have been removed from the practice list due to behaviour (including multiple missed appointments) will not be able to re-register. All new patients will be requested to provide proof of address. This may include a utility bill (mobile telephone not included), rent or mortgage agreement, council tax bill, or solicitor correspondence – all must be dated within the past 3 months.
Violence and abuse to any member of our staff will not be tolerated, and will result in patients being removed from our practice list.
We operate a zero tolerance policy towards violent and abusive behaviour. Unacceptable behaviour that causes harm or the fear of harm to any person within the surgery may lead to the removal of such a patient from the practice list onto the Potentially Violent Patient Scheme.
Many complaints can be resolved by an informal discussion with your Doctor or our Practice Manager. However, we also offer a formal practice complaints procedure and our Practice Manager can give you further information. We always try to provide the best services possible and we try to deal swiftly with any problems that may occur.
